Bereavement Coordinator- Hospice

The Bereavement Coordinator is responsible for planning, implementing, and maintaining a bereavement program to meet the needs of families/caregivers for up to one (1) year following the death of the patient.

**What You’ll Do:**

* Coordinates the assessment and delivery of grief support needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers.
* Provides bereavement information and referral services to callers from the community. Expands and maintains community resource list, as needed.
* Functions independently, but seeks and accepts guidance from others.
* Participates in hospice activities such as in-service education and hospice personnel meetings.
* Organizes annual memorial services for patients who were under our care.
* Oversees the mailings, phone calls, and visits to family/caregivers in the program.
* Functions as part of the IDT team.
* Attend continuing education on dying and the grieving process on an annual basis.

Requirements:

* Strong communication skills and interpersonal skills.
* Valid driver’s license and proof of insurance are required.
* Possess high integrity, including the maintenance of confidential information.
* Minimum high school diploma required. Associate’s degree or higher preferred.
* Upon hire, participate in online Bereavement Coordinator training.
* Must be sensitive to the human needs and situations encountered in the provision of hospice and bereavement care.
