Job Description
**Position Scope:**
The Bereavement Counselor, under the direction of the COO, provides emotional counseling and support to patients, significant others, families, and other team members. Continued management of all aspects of the bereavement program.

**I. Essential Functions/Performance Factors**
· Assesses pre\-bereavement needs of the patient/family at the patient's admission.
o Reviews the grief/loss section on the initial admission form completed by the Social Worker.
o Begins evaluation of family’s bereavement needs at patient’s admission and throughout patient’s stay.
o Evaluates continually effectiveness of intervention and make appropriate changes to Plan of Care.
· Assesses the needs of the bereaved; develops, implements, and periodically evaluates a plan of care.
o Completes initial assessment within agency time frames.
o Develops and implements a Plan of Care related to identified bereaved.
o Evaluates continually effectiveness of interventions and make appropriate changes to Plan of Care.
o Assesses needs of any children/teens in family related to grief and loss issues.
· Educates the bereaved in the understanding of grief as a natural process.
o Provides education regarding grief and mourning during initial visit.
o Supplies on going education/support through every other month mailings.
o Presents opportunities for bereaved to attend grief education classes and special programs.
· Counsels bereaved regarding the effects of grief and loss.
o Pursues interventions identified in the care plan.
o Utilizes agency resources related to coping with grief and loss.
o Follows through with outside referrals when appropriate.
o Works in conjunction with volunteers to provide education/support through quarterly phone calls.
· Provides bereavement support and education to non\-hospice families.
o Interacts on community level by public speaking and networking.
o Provides education to schools, businesses, other organizations and individuals as requested.
o Develops programs for community as needs arise.
· Documents care provided in an accurate, complete, and timely manner.
· Participates in performance improvement activities.
Uses agency resources wisely.

**Education and Work Experience:**
* Bachelor’s Degree in Counseling or Social Work preferred or equivalent experience.
* One to two years of related experience
* Evidence of experience and skills necessary to perform job
* Exhibits excellent communication skills
**Certifications and Licenses:**
Current driver’s license, automobile insurance, and reliable transportation

The Bereavement Counselor must carry out responsibilities in an ethical, professional and confidential manner.

**WORK ENVIRONMENT**
While performing the duties of this job, the employee frequently is exposed to outside weather conditions when traveling between sites. Patients’ home environments are variable with regard to heat, noise, air circulation and conditioning, space, cleanliness, accessibility and all other environmental factors. Inpatient and skilled nursing facility environments are standardized. Office environment in a controlled atmosphere building.
