Care Manager (Licensed)

**Role Overview**
Provides support for care management and coordination activities, collaborating with a multidisciplinary team to deliver integrated member care across various settings. Ensures member progress towards health outcomes and contributes to strategies that promote quality and cost-effective care.


**Key Responsibilities**

* Conducts comprehensive behavioral health assessments and determines eligibility for care coordination or case management.
* Develops and implements individualized care plans with members, caregivers, and healthcare professionals.
* Performs ongoing monitoring, documentation, and evaluation of care plan effectiveness.
* Maintains a caseload for regular outreach and manages service integration across behavioral health, LTSS, and community resources.
* Facilitates interdisciplinary team meetings and uses motivational interviewing to support member engagement.
* Assesses barriers to care and provides coordination assistance; may offer peer consultation and resources.
* Travel may be required for member visits.

**Qualifications & Skills**

* Minimum 2 years healthcare experience, preferably in behavioral health, or relevant education and experience.
* Licensed behavioral health clinician (e.g., LCSW, LMSW, LPC, PhD/PsyD) with active, unrestricted license.
* Valid driver’s license, reliable transportation, and auto insurance for travel.
* Experience working with severe mental health concerns, substance use disorder, or foster care.
* Knowledge of whole person care, chronic conditions, and discharge planning.
* Proficiency with clinical platforms and Microsoft Office; excellent communication skills.

**Preferred Qualifications**

* Certified Case Manager (CCM).
* Experience in behavioral health care management or field-based care.
