Hospice & Beyond Palliative Care LLC is seeking an Experienced Hospice Chaplain who is compassionate, dependable, and committed to providing spiritual and emotional support to patients and families during end-of-life care.

Responsibilities
* Provide spiritual care visits to hospice patients and families based on individual beliefs, values, and preferences
* Complete spiritual assessments upon admission and as needed
* Offer prayer, supportive presence, counseling, and emotional support
* Support patients and families through grief, anticipatory loss, fear, and end-of-life transitions
* Coordinate with nurses, social workers, aides, and physicians as part of the interdisciplinary team
* Participate in IDG meetings and contribute to plan of care
* Provide bereavement support and follow-up when appropriate
* Respect all faith traditions, cultures, and personal beliefs
* Maintain timely and accurate EMR documentation
Requirements
* Chaplaincy training, ministry experience, or equivalent spiritual care background required
* Hospice experience strongly preferred
* Ordination, endorsement, or certification preferred
* Strong communication and counseling skills
* Compassionate bedside manner
* Reliable transportation
* Ability to serve patients of all faiths or no faith tradition
* Bilingual is a plus
