Hospice \& Beyond Palliative Care LLC is seeking an Experienced Hospice Social Worker who is compassionate, dependable, and passionate about helping patients and families during end\-of\-life care.
This is an excellent opportunity for an experienced social worker who values autonomy, independence, flexibility, and meaningful patient impact.
Responsibilities
* Perform psychosocial assessments for hospice patients and families
* Identify emotional, caregiver, financial, housing, and support needs
* Provide counseling, emotional support, and crisis intervention
* Assist with advance directives and community resources
* Participate in IDG meetings and care planning
* Coordinate with nurses, chaplains, physicians, and facilities
* Support bereavement and family coping needs
* Maintain timely EMR documentation
* Educate families on hospice philosophy and services
Requirements
* MSW preferred (BSW considered with relevant hospice experience)
* Texas social work license preferred
* Minimum 2 years hospice experience strongly preferred
* Excellent communication and interpersonal skills
* Reliable transportation
* Knowledge of hospice regulations and documentation standards
* Bilingual is a plus
