As a **PRN Hospice Field RN**, you’ll deliver compassionate, one-on-one care to patients facing life-limiting illnesses - primarily in the place they call home. You’ll have the autonomy to make clinical decisions in the field while being fully supported by a collaborative interdisciplinary team.

This role is ideal for an experienced hospice RN who values flexibility, independence, and deeply meaningful work.

**What You’ll Do**
------------------

* Perform **initial and ongoing comprehensive assessments** of patients’ physical, functional, psychosocial, and environmental needs
* Determine **hospice eligibility, admission, and recertification** using established clinical criteria
* Develop, implement, and adjust **individualized plans of care** to ensure comfort and quality of life
* Provide **education and emotional support** to patients and families on disease progression, end-of-life care, and self-care techniques
* Coordinate with the **interdisciplinary group (IDG)** including physicians, social workers, and chaplains
* Assess caregiver capability and **identify support needs throughout the care journey**
* Supervise and provide clinical direction to **Hospice Aides and LVN/LPNs**
* Document timely and accurate **clinical and progress notes**
* Communicate changes in patient condition and **collaborate on care adjustments**

**What You’ll Need**
--------------------

* Active **Registered Nurse (RN) license**
* **CPR certification**
* Valid **driver’s license and auto insurance**
* Minimum **1 year of hospice experience preferred** (2+ strongly preferred)
* Experience with **Homecare Homebase** is a plus
