Hospice Certified Nursing Assistant
Job Description
**Role Overview:**
A compassionate Hospice Certified Nursing Assistant is sought to provide personal care, emotional support, and assistance with daily activities to hospice patients and their families.
**Key Responsibilities**
* Deliver personal care such as bathing, grooming, oral hygiene, and repositioning.
* Assist with activities of daily living in patient homes or care facilities.
* Monitor and report patient condition changes, vital signs, and comfort levels.
* Provide emotional support and companionship to patients and families.
* Help with light housekeeping, meal prep, and personal laundry related to patient care.
* Accurately document care activities and collaborate with the interdisciplinary team for holistic care.
**Qualifications & Skills**
* Current CNA certification in Illinois.
* Experience in hospice, palliative, or end-of-life care preferred.
* Strong understanding of patient-centered, comfort-focused care principles.
* Excellent communication and interpersonal skills.
* Reliable transportation and valid driver’s license for home visits.
* Ability to work independently with compassion, patience, and professionalism.
* CPR certification and ability to meet health screening requirements.
