At First Choice Home Health \& Hospice, we are dedicated to delivering compassionate, person\-centered care that supports patients and families during the most meaningful moments of life. We are currently seeking a compassionate and experienced *Hospice Pastoral Care Counselor* to join our interdisciplinary team.
**Position Summary:**
The Hospice Pastoral Care Counselor provides spiritual, emotional, and bereavement support to hospice patients, families, and staff. This role is an integral part of our interdisciplinary team, ensuring that patients and loved ones receive holistic care that respects their spiritual needs, values, and traditions.
**Key Responsibilities:**
* Conduct timely spiritual assessments upon admission and develop individualized spiritual care plans.
* Provide pastoral counseling and emotional support to patients, families, and staff.
* Facilitate prayers, worship services, rituals, or spiritual practices as requested.
* Respectfully support all faiths, philosophies, and personal belief systems.
* Collaborate with local clergy and spiritual leaders when requested by the patient or family.
* Participate in interdisciplinary group (IDG) meetings and care planning discussions.
* Document all services and counseling activities in the patient’s clinical record.
* Develop and deliver bereavement care plans and offer grief support before and after a patient’s death.
* Support staff through counseling, education, and spiritual guidance as needed.
* Maintain ongoing contact with families during the bereavement period.
* Ensure services align with patient and family needs and evaluate satisfaction with care.
**Qualifications:**
* Ordained, commissioned, or credentialed by an organized religious group.
* At least 3 years of experience providing spiritual care in a hospice or end\-of\-life setting.
* Experience with bereavement counseling strongly preferred.
* Excellent communication, listening, and interpersonal skills.
* Basic computer literacy and documentation skills.
* Physically able to climb stairs and remain on your feet for extended periods.
* Valid driver’s license, current auto insurance, and satisfactory driving record.
* Must complete at least 12 hours of continuing education annually.
