**Position Summary**
The Field Clinical Performance Manager – Hospice provides clinical leadership, oversight, and performance management for hospice field nurses. This role ensures the delivery of compassionate, patient\-centered end\-of\-life care while maintaining regulatory compliance and clinical excellence.
The Manager is responsible for recruitment, onboarding, education, mentorship, and ongoing competency validation of hospice nursing staff. Through field supervision, quality monitoring, and professional development initiatives, this position supports a culture of dignity, comfort, and excellence in hospice care.
**Key Responsibilities**
**Clinical Recruitment \& Onboarding**
* Partner with Human Resources to interview and select qualified hospice RNs and LPNs/LVNs.
* Assess candidates’ clinical expertise, hospice philosophy alignment, and communication skills.
* Lead structured orientation programs focused on hospice philosophy, symptom management, and interdisciplinary collaboration.
* Validate competency in end\-of\-life care before assigning patients independently.
**Hospice Clinical Education \& Training**
* Develop and deliver training programs specific to hospice and palliative care, including:
+ Pain and symptom management
+ End\-of\-life care planning
+ Medication management in terminal illness
+ Comfort\-focused interventions
+ Psychosocial and spiritual support principles
+ Family education and bereavement support
+ Hospice documentation and care planning
* Conduct case reviews, interdisciplinary teaching sessions, and competency evaluations.
* Provide targeted remediation and coaching when clinical gaps are identified.
**Field Supervision \& Performance Oversight**
* Conduct supervisory visits in patient homes, assisted living facilities, and nursing facilities.
* Observe and evaluate clinical practice to ensure compassionate, high\-quality care delivery.
* Provide real\-time coaching, mentorship, and emotional support to hospice clinicians.
* Monitor documentation accuracy, timeliness, and compliance standards.
* Develop performance improvement plans as necessary.
**Quality Assurance \& Regulatory Compliance**
* Ensure compliance with state and federal hospice regulations, including Medicare Conditions of Participation for hospice.
* Participate in Quality Assessment and Performance Improvement (QAPI) initiatives.
* Audit clinical documentation for regulatory adherence and quality metrics.
* Identify trends in symptom management, hospitalizations, and care transitions to guide education initiatives.
* Stay current on evolving hospice standards and best practices.
**Interdisciplinary Collaboration**
* Partner with physicians, social workers, chaplains, hospice aides, and bereavement coordinators.
* Support interdisciplinary team meetings and promote collaborative care planning.
* Serve as a clinical resource for complex or high\-acuity hospice cases.
**Professional Development \& Staff Support**
* Foster resilience, compassion, satisfaction, and professional growth among hospice nurses.
* Coordinate continuing education opportunities in palliative and end\-of\-life care.
* Support leadership development and succession planning.
* Promote staff retention through mentorship and supportive leadership practices.
**Qualifications**
* Active Registered Nurse (RN) license in good standing.
* Bachelor of Science in Nursing (BSN) required; Master’s preferred.
* Minimum 3–5 years of hospice or palliative care experience.
* Prior experience in clinical supervision, education, or leadership preferred.
* Strong knowledge of hospice regulations and Medicare Conditions of Participation.
* Demonstrated ability to mentor and evaluate clinical staff.
* Excellent communication, emotional intelligence, and leadership skills.
* Ability to travel locally for field supervision.
