Hospice Liaison Nurse - Weekends

We are seeking a
**Registered Nurse (RN) Hospice Liaison**
to serve patients and families by facilitating hospice referrals and admissions.


**Key responsibilities**
include assessing patient eligibility, educating referral sources, coordinating admissions, and supporting families through end-of-life care decisions. The role involves collaborating with physicians, hospital staff, and community agencies to ensure seamless transitions to hospice services, ordering medications and supplies, and respecting patients' cultural and religious preferences.


**Qualifications**
require an accredited nursing degree, active RN license (multi-state preferred), and BLS certification. A minimum of 2 years in acute care is preferred. Candidates should demonstrate strong communication, critical thinking, time management, and teamwork skills, with proficiency in technology and documentation.


**Core duties**
include patient and family education, determining care goals, reviewing medical records, coordinating legal and transportation arrangements, and ensuring compliance with professional and ethical standards.
