Hospice Licensed Counselor LCPC LCSW

We Are Hiring: **Location:** Elk Grove, IL**Facility:** Ascension Hospice**Department/Specialty:** Hospice Patient Care Svcs**Schedule:** Day Shift \| Full Time \| 40 hrs week with some evenings fro running group sessions.**Salary:** $63,363\.30 \- $92,688\.83 per year What You Will Do: * Provide behavioral health counseling, including individual and group therapy, based on patient assessments and established treatment goals. * Maintain accurate, confidential clinical documentation and evaluate patient progress to adjust therapeutic interventions as needed. * Educate patients on community resources and therapeutic activities to support ongoing recovery. * Collaborate with patients, families, physicians, and community providers to ensure coordinated care from admission through discharge. What You Will Need: Licensure / Certification / Registration:* One or more of the following required: + Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. + Therapist specializing in Marriage \& Family credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. + Licensed Clinical Social Worker (LCSW) credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or transfer date. + Licensed Clinical Professional Counselor (LCPC) credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. + Social Worker specializing in Clinical Practice credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. * BLS Provider preferred. American Heart Association or American Red Cross accepted. Education:* High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
