Hospice RN - After Hours On-Call (weekends only)

**Job Summary** We are seeking a dedicated and compassionate Registered Nurse to join our healthcare team. The ideal candidate will provide high\-quality patient care in various settings, including patient homes and assisted living facilities. This role requires a strong commitment to patient advocacy, effective communication skills, and the ability to work collaboratively with multidisciplinary teams. Our offices are located in Allen, TX but this on\-call position will cover **all areas of the Metroplex** in need of RN coverage: **Responsibilities** * Assess and monitor patient health status, including vital signs and medical history. * Administer medications and treatments as prescribed by physicians. * Develop and implement individualized care plans for patients, particularly those with dementia or disabilities. * Provide education and support to patients and their families regarding health management, including family planning options. * Utilize electronic health record systems (FireNote) for documentation and patient management. * Collaborate with other healthcare professionals in hospice medicine to ensure comprehensive care delivery. * Maintain a safe environment for patients by adhering to infection control protocols and safety guidelines. **Skills** * Proficiency in using electronic health record systems (FireNote). * Experience in dementia care and working with individuals with disabilities. * Knowledge of family planning practices, bereavement and patient education techniques. * Excellent communication skills for effective interaction with patients, families, and healthcare teams. * Ability to work in fast\-paced environments while maintaining attention to detail. * Compassionate approach towards patient care with a focus on empathy and support.
