Hospice RN Case Manager - Field / Visiting Nurse ( Kane & McHenry Counties)

Local Hospice Company is looking for a compassionate and dedicated Hospice RN Case Manager to join our field-based team in Saint Charles, IL. In this rewarding visiting nurse role, you'll be at the heart of delivering exceptional end-of-life care directly to patients and families in the comfort of their own homes. **Responsibilities:**

* Manage a caseload of hospice patients, conducting in-home assessments and developing individualized care plans
* Perform skilled nursing visits to monitor patient symptoms, pain management, and overall comfort
* Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and aides to coordinate holistic patient care
* Educate and support patients and their families on disease progression, medication management, and comfort care techniques
* Document clinical findings, care plans, and visit notes accurately and in compliance with hospice regulations
* Ensure compliance with Medicare/Medicaid hospice guidelines and agency policies
* Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
* Participates in on-call duties as defined by the on-call policy.
* Ensures that arrangements for equipment and other necessary items and services are available.
* Attends bi-monthly interdisciplinary group meetings.

**Requirements:**

* Active RN license in the state of Illinois (required)
* Minimum 1-2 years of clinical nursing experience; hospice, palliative care, or home health experience preferred
* Strong knowledge of pain and symptom management in end-of-life care
* Maintains a current CPR certification.
* Excellent communication and interpersonal skills with patients, families, and care teams
* Ability to work independently in a field-based, self-directed environment
* Valid driver's license and reliable transportation for home visits in the Saint Charles, IL area
* Proficiency with electronic medical records (EMR) documentation
* Empathetic, patient-centered demeanor with a commitment to dignity and compassionate care

Pay: $55.00 - $100.00 per hour

Expected hours: 14.0 – 22.0 per week

Work Location: On the road
