Hospice RN Case Manager - Full Time/Salary - WEST VALLEY (No Weekends/Oncall!)

**Hospice Registered Nurse (RN) Case Manager**

As a Hospice RN Case Manager, you will primarily provide in-home case management for hospice patients while occasionally completing hospice admissions to help maintain continuity of care and support your assigned caseload.

**What You'll Do**

* Manage and coordinate hospice care for an assigned patient caseload
* Perform comprehensive nursing assessments in patients' homes, assisted living communities, group homes, and skilled nursing facilities
* Develop, implement, and update individualized plans of care
* Educate patients, families, and caregivers on disease progression, symptom management, medications, and end-of-life care
* Collaborate with physicians, social workers, chaplains, aides, and other members of the interdisciplinary team
* Monitor changes in patient condition and communicate clinical updates as needed
* Complete accurate and timely clinical documentation
* Periodically conduct hospice admissions and evaluate patient eligibility for hospice services
* Ensure compliance with hospice regulations and Medicare guidelines
* Provide compassionate support to patients and families throughout their hospice journey

**Qualifications**

* Current Arizona Registered Nurse (RN) license
* Graduate of an accredited nursing program (Associate or Bachelor's degree)
* Minimum of two years of RN experience in a clinical setting
* Hospice, home health, palliative care, or case management experience ***strongly preferred***
* Current CPR certification
* Valid driver's license and reliable transportation
* Strong assessment, communication, and organizational skills
* Comfortable using electronic medical records and computer-based documentation systems (HCHB experience is preferred)
