**Hospice RN Case Manager – $85,000–$100,000 | Mon–Fri | ~15-Patient Caseload**

**Location:** Bucks and Montgomery County
**Schedule:** Monday–Friday | 8:00 AM–5:00 PM
**On-Call:** Rotating | Limited occasional evenings/weekends
**Caseload:** Approximately 15 patients (subject to change based on census and acuity)

**Your Role**
-------------

As an RN Case Manager, you are the clinical lead for your assigned patients - supported by a strong interdisciplinary team.

You will:

* Conduct comprehensive nursing assessments (with Admissions Nurse support)
* Develop and manage individualized plans of care
* Collaborate with physicians, LPNs, social workers, chaplains & aides
* Educate families on symptom management and disease progression
* Participate in IDT meetings
* Maintain timely, compliant documentation

**Qualifications**
------------------

* Active Pennsylvania RN License
* Hospice or home health experience strongly preferred
* Strong clinical judgment & communication skills
* Valid driver's license & reliable transportation
