Our mission to become the 'Employer of Choice' has led up to wining a 'Top Workplace' Award by the Indy Star for the past 5 years, and we are not finished yet! ### **Job Description** * Full\-time Hospice RN Case Manager position * Schedule: Monday thru Friday, 8a\-5p * Available territory openings: Case Manager typically assigned to cover 1\-2 counties near the Office. * Performs physical and psychosocial assessment on all hospice patients. * Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. * Responds to the physical, mental, social and emotional needs of terminally ill patients and their families. * Assesses patient’s condition, initiates plan of care, re\-evaluates and updates as necessary. * Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate. * Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc ### **Benefits** * Competitive salary * Mileage reimbursement * Excellent PTO program (up to 136 hours in first year) * Every Friday is payday! * Health, dental, vision * 401k match program * Great team environment! ### **Requirements** * One year Hospice experience preferred * 2 years of RN experience required * Licensed Registered Nurse in State of Indiana * Current driver's license * Must have reliable transportation and insurance Salaried poisition \+ mileage reimbursement \+ Back\-up On\-Call Stipends (\+visit pay if applicable) We hope to speak with you about the rewarding nature of hospice nursing!
