Hospice RN Case Manager (Per Diem) - Sonoma County

**Hospice RN Case Manager | Sonoma County | Per Diem with a Path to Part-Time or Full-Time**

Crossroads Home Health & Hospice is building a new hospice team in Sonoma County, and we want experienced RN Case Managers in on the ground floor. You will care for hospice patients and families across Santa Rosa, Petaluma, Rohnert Park, Windsor, Sebastopol, Healdsburg and Sonoma, set your own visit schedule, and work with a clinical leadership team that answers the phone. Start per diem on your terms, then grow into a part-time or full-time role as the census builds.

**Pay and Benefits**

* $55 to $65 per hour
* Mileage reimbursed at $0.76 per mile

**What You Will Do**

* Manage a caseload of hospice patients in their homes and in facilities
* Complete admission, routine, recertification and death visits
* Assess and manage pain and symptoms, and update the plan of care with the hospice physician and IDT
* Educate and support patients and families through end of life
* Coordinate with the hospice aide, MSW, chaplain and volunteers
* Participate in IDG meetings
* Document visits accurately and on time in the EMR

**What We Are Looking For**

* Active, unrestricted California RN license
* Hospice experience strongly preferred; home health, oncology, palliative or acute care RNs with strong assessment skills are encouraged to apply
* Current CPR certification
* Reliable vehicle, valid driver's license and auto insurance
* Ability to pass a background check
* Comfort working independently in the field and charting in an EMR

Pay: $55.00 - $65.00 per hour
