Job Description
Hospice – RN Case Manager
The Hospice RN Case Manager is responsible for coordinating all aspects of patient care for individuals receiving hospice services. This includes assessment, planning, education, support, and evaluation to ensure the highest quality of care. The RN Case Manager works closely with the interdisciplinary team to deliver compassionate, patient-centered services in accordance with hospice standards and regulatory guidelines.
Key Responsibilities
Provide comprehensive nursing assessments, develop individualized care plans, and ensure timely follow-up.
Deliver skilled nursing care focused on symptom management, pain control, and comfort measures.
Coordinate care with physicians, social workers, chaplains, CNAs, and other interdisciplinary team members.
Educate patients and families on the hospice philosophy, disease progression, symptom management, and medication usage.
Maintain accurate, timely, and compliant documentation in the EMR system.
Participate in on-call rotations as assigned.
Communicate changes in condition promptly and effectively to the healthcare team.
Provide emotional support and advocacy for both patients and their caregivers.
Ensure care is delivered in alignment with all federal, state, and agency regulations.
Qualifications
Valid Georgia RN License (Required)
Valid Driver’s License & Reliable Transportation (Required)
Strong assessment, communication, and clinical judgment skills
Ability to work independently while collaborating effectively with a team
Mileage Reimbursement
