Job Description

**Registered Nurse (RN) – Hospice**
**Cleveland | Flexible Compensation Options**

Choose the compensation package that works best for you:

* $80,000 salary + full family benefits
* Up to $115,000 salary with no benefits

Join our growing hospice team and provide one-on-one patient care while working independently in communities across Cleveland.

We're looking for a compassionate, dependable Registered Nurse (RN) who wants more autonomy, meaningful patient relationships, and an alternative to the traditional hospital environment. This position is ideal for an RN who enjoys managing their own day, getting to know patients and families, and providing high-quality, comfort-focused care where patients feel most at home.

**Why Join Us?**

* Compensation up to $115,000 annually
* Choice between a benefits package or higher cash compensation
* Full family benefits available
* Monday–Friday daytime schedule, plus a shared after-hours and weekend rotation
* **Extra compensation for after-hours and weekend rotation shifts**
* Work independently and manage your daily patient schedule
* One-on-one patient care
* No hospital floor assignments
* Local Cleveland territory
* Supportive clinical and administrative team
* The opportunity to make a meaningful difference for patients and families

**Responsibilities**

As a Hospice RN, you will:

* Perform comprehensive hospice assessments and develop individualized plans of care focused on comfort and quality of life.
* Provide skilled hospice nursing visits in patients' homes, assisted living communities, and nursing facilities as assigned.
* Manage pain and end-of-life symptoms such as nausea, dyspnea, agitation, and anxiety, adjusting care in collaboration with the hospice physician.
* Provide wound care, medication management, and other comfort-focused skilled nursing interventions per the plan of care.
* Educate and support patients and families on disease progression, the dying process, and symptom management.
* Coordinate with the interdisciplinary team (physician, social worker, chaplain, home health aide, volunteers, and bereavement counselor) to meet patients' and families' physical, emotional, and spiritual needs.
* Communicate condition changes to the physician and team, including signs of active dying.
* **Take part in the after-hours and weekend rotation, including phone triage, visits, and death pronouncement as needed.**
* Complete timely clinical documentation supporting hospice eligibility, and participate in IDG care planning meetings.
* Maintain compliance with Medicare hospice regulations, HIPAA, and agency policies.

**Qualifications**

* Current and unrestricted Ohio Registered Nurse (RN) license
* Current BLS/CPR certification
* At least 1 year of hospice experience
* Additional experience in home care, hospitals, skilled nursing, rehabilitation, assisted living, or other clinical settings is welcome
* Ability to take part in the after-hours and weekend rotation
* Strong assessment and critical-thinking skills
* Excellent communication and patient- and family-education skills
* Comfortable working independently in patients' homes
* Reliable transportation and the ability to travel within the assigned territory
* Strong organizational and time-management skills
* Comfortable using electronic medical record systems and mobile technology

**A Better Alternative to the Hospital**

If you're an RN who's ready for a change from long hospital shifts, constant interruptions, and caring for multiple patients at once, hospice offers a different way to practice nursing. You'll spend meaningful one-on-one time with patients and families, use your clinical judgment independently, and help patients stay comfortable and supported at home through the end of life.

Whether you value family benefits and stability or prefer maximum cash compensation, we offer a compensation structure that gives experienced nurses more choice. Rotation shifts come with extra pay on top of either package.
