**About the Role:**

Solara Hospice LLC is looking for a compassionate and dedicated Hospice RN to join our team in Braintree, MA. In this meaningful role, you'll provide expert end-of-life care and emotional support to patients and their families during life's most important moments. If you're a skilled nurse with a heart for hospice, we'd love to have you on our team.

**Responsibilities:**

* Conduct comprehensive patient assessments and develop individualized plans of care for hospice patients
* Provide skilled nursing visits in home, facility, and residential settings across the Braintree, MA area
* Manage pain and symptom control using evidence-based palliative care practices
* Educate and support patients, families, and caregivers on disease progression and end-of-life care
* Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and aides
* Document clinical findings, care plans, and patient updates in accordance with Medicare/Medicaid hospice guidelines
* Provide on-call nursing support and respond to urgent patient needs as needed

**Requirements:**

* Active RN license in the state of Massachusetts (required)
* Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
* Strong knowledge of pain management, symptom control, and end-of-life care protocols
* Excellent communication and interpersonal skills with patients, families, and care teams
* Ability to work independently, manage a caseload, and prioritize in a fast-paced environment
* Valid driver's license and reliable transportation for field-based visits
* Proficiency with electronic medical records (EMR) documentation
