**About the Role:**
Solara Hospice LLC is looking for a compassionate and dedicated Hospice RN to join our caring team in Mattapan, MA. In this meaningful role, you'll provide expert clinical care and emotional support to patients and families during life's most important moments. If you're a nurse who leads with heart and skill, we'd love to have you on board.
**Responsibilities:**
* Conduct comprehensive patient assessments and develop individualized hospice care plans
* Provide skilled nursing care to terminally ill patients in home or facility settings
* Administer and manage pain control and symptom management protocols
* Educate and support patients, families, and caregivers on end-of-life care
* Collaborate with interdisciplinary hospice team including physicians, social workers, and chaplains
* Document patient care, progress notes, and clinical updates in compliance with hospice regulations
* Respond to after-hours patient and family needs as part of on-call rotation
**Requirements:**
* Active RN license in the state of Massachusetts
* Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
* Strong knowledge of pain management and end-of-life symptom control
* Excellent communication and interpersonal skills with patients and families
* Ability to work independently and manage a patient caseload efficiently
* Valid driver's license and reliable transportation for home visits
* Compassionate, empathetic, and emotionally resilient demeanor
