Hospice Social Worker - LCSW or MSW
Job Description
* Schedule: Monday-Friday | 8:00 AM-5:00 PM
* Hutchinson, KS and surrounding areas
**Hospice Social Workers offer care and assistance to individuals in the final stage of life, as well as their families and close ones.**
Their purpose is to provide emotional and practical support, with the goal of enhancing the well-being of those confronting end-of-life challenges. Hospice social workers collaborate closely with healthcare teams to organize treatments, services, and available resources, ultimately aiming to enhance a patient's overall quality of life.
**Requirements**
* Master's in Social Work (MSW) or LCSW required
* Hospice experience preferred
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Professional, dependable, and organized
**Responsibilities**
* Provide counseling and support to patients and families in the home
* Assist with advance directives and community resources
* Participate in the hospice interdisciplinary team (IDG)
* Coordinate referrals and additional support services
* Maintain positive relationships with patients, families, staff, and referral partners
