Hospice Social Worker (LMSW/LCSW)

**Role Overview:**
A Social Worker provides psychosocial support and counseling to patients and families facing terminal illness, collaborating with the interdisciplinary team to develop and update care plans. They assess psychosocial and financial needs, offer guidance on benefits, and assist with coping strategies related to grief, loss, and emotional stresses.


**Key Responsibilities**

* Conducts psychosocial assessments and history taking
* Provides crisis intervention, suicide assessment, and grief counseling
* Locates and coordinates resources for financial, social, and emotional support
* Documents services accurately and maintains medical records
* Offers bereavement support and family counseling
* Trains and supervises volunteers and supports staff education
* Participates in discharge planning and ongoing education
* Acts as a consultant to staff and coordinates patient care under supervision
* Ensures understanding of hospice services, ethical issues, and cultural diversity

**Qualifications & Skills**

* Master's Degree in Social Work (MSW) required
* Preferred one year of experience in social work
* Current licensure and valid driver’s license
* Strong assessment, counseling, crisis intervention, and communication skills
* Ability to work flexibly, maintain professionalism, and foster compassionate interactions
