Hospice Social Worker
Job Description
**Role Overview:**
A compassionate
*Social Worker*
is needed to provide holistic end-of-life care, working with patients experiencing life-limiting illnesses and supporting their families.
**Key Responsibilities**
* Assess emotional, social, and practical needs of patients and families.
* Develop and implement personalized care plans in collaboration with interdisciplinary teams.
* Offer counseling and emotional support to patients and families during challenging times.
* Coordinate resources and services to enhance quality of life.
* Advocate for patients’ needs within the healthcare team and community.
**Qualifications & Skills**
* Strong empathetic and compassionate nature.
* Excellent communication and interpersonal skills.
* Ability to collaborate effectively with healthcare professionals.
* Experience in social work, preferably in end-of-life or palliative care.
* Knowledge of social services, emotional support techniques, and ethical considerations in hospice care.
