Concho Hearts Hospice is seeking a compassionate PRN Medical Social Worker (LMSW or LBSW) to support patients and families in the Wichita Falls area.

Hospice social work is about more than assessments; it’s about helping families navigate one of life’s most difficult journeys with support, guidance, and compassion. Our Social Workers play a vital role in providing emotional support, connecting families with resources, and helping patients maintain dignity and comfort.

**What You’ll Do:**

* Assess the psychosocial needs of hospice patients and their families.
* Ability to provide compassionate emotional support to patients and families during end-of-life care.
* Assist families in identifying community resources and financial or social support services.
* Collaborate with the RN Case Manager and interdisciplinary team to develop individualized care plans.
* Participate in interdisciplinary team meetings (IDT) to support patient care.
* Assist with bereavement planning and coordinate with the Bereavement Coordinator as needed.
* Document assessments and interventions in the EMR system.

**What We’re Looking For:**

* Licensed Social Worker in Texas (LMSW or LBSW).
* Master’s or Bachelor’s degree in Social Work from an accredited program.
* Minimum 1 year of social work experience in healthcare, hospice, or long-term care.
* Hospice experience preferred but not required.
* Strong communication and counseling skills.
* Ability to work independently while collaborating with a care team.
* Reliable transportation and willingness to travel locally.
