This role is ideal for dedicated professionals passionate about enhancing quality of life through compassionate social work and clinical expertise. **JOB SUMMARY:** · The Hospice Social Worker is essential for meeting the psychosocial needs of hospice patients and their families, including assisting with end of life planning, support for options at time of death, and assisting with options as patient care needs increase. · The Hospice Social Worker – Navigator serves as both a core member of the Interdisciplinary Group (IDG) and as a primary relationship\-builder for patients and families from the first point of contact. In this role, the Social Worker may serve as the initial outreach contact following referral, begin building rapport with the patient and family prior to admission, and remain actively engaged throughout the hospice stay. · This position integrates psychosocial assessment, emotional support, resource coordination, and IDG collaboration to ensure holistic, patient\-centered care consistent with Solterra Hospice’s mission, philosophy, and Medicare Conditions of Participation. **REQUIRED EDUCATION AND EXPERIENCE:** · Must be a graduate of an accredited social work program · Must be an LMSW, CSWA, or LCSW in the state of Oregon · Must have experience working with death and dying · Hospice experience preferred **REQUIRED KNOWLEDGE, SKILLS, \& ABILITIES:** · Must be familiar with the philosophy of hospice and understand the needs of the terminally ill · Must have knowledge of community resources · Must have excellent interpersonal skills · Must possess the ability to deal with stressful situations, as well as grieving families · Must demonstrate the ability to provide professional and courteous interactions with patients, their families, and staff under sensitive circumstances **OTHER REQUIREMENTS:** · Must be dependable and flexible in work schedule · Must have a team\-oriented approach to employment **ESSENTIAL JOB FUNCTIONS:** **Navigator Responsibilities:** * Serve as initial point of contact for patients and families when assigned * Conduct first outreach call following referral when appropriate * Begin establishing therapeutic relationship prior to admission * Provide emotional support and education about hospice services * Attend admission visits when appropriate to support transition * Introduce and begin integration of IDG team members from first contact * Maintain consistent involvement throughout the hospice stay **Clinical \& Psychosocial Responsibilities:** * Complete comprehensive psychosocial assessments per CMS timelines * Develop and update individualized care plans with measurable goals * Provide counseling, grief support, and crisis intervention * Facilitate family meetings and conflict resolution as needed * Assist with advance care planning discussions * Coordinate community resources and referrals * Participate actively in IDG meetings and documentation **Bereavement \& Continuity Responsibilities:** * Ensure continuity of support into bereavement phase when appropriate * Participate in bereavement risk assessment and planning * Support smooth transition from active care to bereavement services **WORK ENVIRONMENT:** · Must comply with all policies and procedures, including following processes as defined by Hospice rules and regulations · Must comply with harassment and discrimination policies · Must foster good working relationships with hospice team and community members, in addition to patients and families · Must be able to read, write, and speak in English · Must be a licensed driver with licensed automobile that is insured, in accordance with state and local requirements, or have an arrangement in place with someone who meets these requirements that will reliably be providing chauffer/private driving services. · Must be willing to submit to a criminal background check prior to hire, and annually thereafter Job Type: Full\-time Pay: $38\.00 \- $42\.00 per hour Expected hours: 40 per week Benefits: * 401(k) * Health insurance * Paid time off * Vision insurance Work Location: Hybrid remote in Salem, OR 97301
