**Three Rivers Hospice** is seeking a compassionate and dedicated **Hospice Social Worker** to join our interdisciplinary team serving **Mount Vernon, Missouri and the surrounding communities**.

As a Hospice Social Worker, you will play a vital role in helping patients and families navigate the emotional, social, and practical challenges of end-of-life care. You’ll advocate for each patient’s wishes, provide compassionate counseling, connect families with valuable community resources, and help ensure every patient receives care with dignity and respect.

REQUIRED EDUCATION – PLEASE READ BEFORE APPLYING

Applicants **MUST have one of the following degrees:**

* **Bachelor’s Degree in Social Work (BSW)**, OR
* **Master’s Degree in Social Work (MSW)**

**No other degrees will be accepted for this position. Applicants without a Bachelor’s or Master’s Degree specifically in Social Work will not be considered.**

Responsibilities

* Complete comprehensive psychosocial assessments and develop individualized plans of care.
* Provide emotional support, counseling, and grief support to patients and their families.
* Advocate for each patient’s goals, values, and end-of-life wishes.
* Assist patients and caregivers in identifying community resources, financial assistance, and support services.
* Collaborate with physicians, nurses, chaplains, aides, volunteers, and the interdisciplinary hospice team to provide holistic, patient-centered care.
* Support patients and families through crisis situations using effective intervention and counseling techniques.
* Maintain accurate and timely documentation, including assessments, care plans, and progress notes.
* Travel throughout the **Mount Vernon territory** to meet the needs of patients and families.

Qualifications

* **Bachelor’s Degree in Social Work (BSW) or Master’s Degree in Social Work (MSW) – REQUIRED**
* Strong communication, counseling, and interpersonal skills.
* Compassionate and professional approach to patients and families.
* Ability to work independently while collaborating effectively with an interdisciplinary team.
* Reliable transportation and willingness to travel throughout the assigned territory.
