Hospice Social Worker

We are seeking a compassionate and dependable Hospice Social Worker (MSW) to join our interdisciplinary hospice team. The Social Worker will provide psychosocial support, counseling, education, and resource coordination to hospice patients and their families throughout the end-of-life care journey.

**Responsibilities:**

* Complete psychosocial assessments and develop individualized plans of care
* Provide emotional support and counseling to patients and families
* Assist families with community resources, financial concerns, advance directives, and end-of-life planning
* Participate in interdisciplinary group (IDG) meetings and collaborate with nurses, physicians, chaplains, and other team members
* Assist with crisis intervention and complex family or psychosocial situations
* Coordinate community resources and referrals as appropriate
* Maintain timely and accurate clinical documentation
* Support patients and families through changes in condition, caregiving needs, and bereavement concerns
* Ensure services are provided in accordance with hospice policies and applicable regulatory requirements

**Qualifications:**

* Master’s degree in Social Work (MSW) from an accredited program
* Current state licensure or eligibility as applicable
* Hospice, home health, palliative care, or healthcare experience preferred
* Knowledge of community resources and end-of-life care preferred
* Strong communication, organizational, and interpersonal skills
* Ability to work independently in a field-based environment
* Valid driver’s license, reliable transportation, and current auto insurance
