Hospice Spiritual Care Counselor

We are seeking a compassionate and dedicated Hospice Spiritual Counselor/Chaplain to join our interdisciplinary hospice team. This position provides spiritual and emotional support to hospice patients and their families while respecting each individual’s personal beliefs, values, culture, and preferences.

**Responsibilities:**

* Complete spiritual assessments for hospice patients and families
* Develop and implement individualized spiritual plans of care
* Provide spiritual counseling, emotional support, prayer, and supportive presence based on patient and family preferences
* Provide support to individuals of all faiths, beliefs, and cultural backgrounds
* Coordinate with community clergy and spiritual resources when requested
* Participate in interdisciplinary group (IDG) meetings and collaborate with the hospice care team
* Provide support during times of crisis, decline, death, and bereavement
* Assist with memorial services or other spiritual services as appropriate
* Maintain timely and accurate clinical documentation
* Follow hospice policies and applicable regulatory requirements

**Qualifications:**

* Degree in theology, divinity, pastoral studies, counseling, or a related field preferred
* Ordination, endorsement, chaplaincy training, or equivalent spiritual care experience preferred
* Hospice, palliative care, hospital, or healthcare experience preferred
* Ability to provide nonjudgmental spiritual support to patients and families with diverse beliefs
* Excellent communication and interpersonal skills
* Ability to work independently and as part of an interdisciplinary team
* Valid driver’s license, reliable transportation, and current auto insurance for patient visits
