**Job Title: Spiritual Counselor (Per Diem)**
**Location:** Newport Hospice
**Reports To:** Clinical Director / Director of Patient Care Services
**Service Locations:** Inland Empire
Position Summary
Newport Hospice is seeking a compassionate and dedicated Spiritual Counselor to provide spiritual, emotional, and bereavement support to hospice patients, families, caregivers, and staff. The Spiritual Counselor is an integral member of the interdisciplinary hospice team and is responsible for assessing and addressing the spiritual needs of patients and families while respecting diverse faith traditions, beliefs, cultures, and values.
Essential Duties and Responsibilities
* Conduct spiritual assessments and develop individualized plans of care based on patient and family needs.
* Provide spiritual counseling, guidance, support, and presence to patients and families coping with serious illness, end\-of\-life issues, grief, loss, and bereavement.
* Facilitate discussions regarding spiritual concerns, meaning, hope, forgiveness, life review, and end\-of\-life preparation.
* Coordinate and collaborate with community clergy, faith leaders, and spiritual resources as requested by patients and families.
* Participate as an active member of the interdisciplinary team, contributing to care planning and patient case conferences.
* Document all patient visits, assessments, interventions, and care coordination activities in the electronic medical record in accordance with hospice regulations and agency policies.
* Provide support and education to hospice staff regarding spiritual care issues and cultural sensitivity.
* Assist with memorial services, bereavement activities, and other hospice\-sponsored events as needed.
* Maintain patient confidentiality and comply with all HIPAA requirements.
* Support the hospice philosophy of care, emphasizing comfort, dignity, quality of life, and patient\-centered care.
* Participate in orientation, ongoing training, staff meetings, and quality improvement activities.
Qualifications
* Graduate of an accredited theological, divinity, pastoral care, or related program preferred.
* Ordination, ecclesiastical endorsement, or recognized standing within a faith tradition preferred.
* Minimum of one (1\) year of experience in hospice, palliative care, healthcare chaplaincy, pastoral ministry, counseling, or related field preferred.
* Knowledge of hospice philosophy, end\-of\-life care, grief and bereavement processes, and spiritual assessment practices.
* Ability to provide care and support to individuals of all faith traditions, cultures, and belief systems.
* Excellent interpersonal, communication, and active listening skills.
* Ability to work independently while collaborating effectively with an interdisciplinary team.
* Valid driver's license, reliable transportation, and proof of automobile insurance.
* Ability to travel within the hospice service area.
Physical Requirements
* Ability to travel to patient residences, assisted living facilities, skilled nursing facilities, and hospitals.
* Ability to sit, stand, bend, and walk for extended periods.
* Ability to lift up to 25 pounds occasionally.
