**Lakeshore Community Liaison – Faith Hospice, full\-time** Faith Hospice has a unique opportunity for a Community Liaison to join their amazing team! This professional works 40 hours per week identifying and educating referral sources, primarily in physician offices in communities along the Lakeshore for Faith Hospice. The services of Faith Hospice include community hospice, our beautiful in\-patient hospice residence called Trillium Woods, and palliative care services. **Why Join Faith Hospice? At Faith Hospice, we utilize highly skilled nurses, medical social workers, chaplains, home health aides and volunteers to serve our patients, from medical and emotional care to pain management and bereavement counseling. **What Will You Do?** The Community Liaison is a key member of the team: * Build referral volume for all of the Faith Hospice services, building working relationships with key personnel at physician offices and maintain schedule of on\-site meetings * Maintain knowledge of services and programs offered through Faith Hospice and education referral sources accordingly * Develop campaign materials to supplement formal marketing collateral * Represent Faith Hospice in the community through direct involvement, presentations, vendor fairs and other activities * Serve as referral source advocate by identifying needs and facilitating problem resolution * Maintain and utilize the marketing database **Skills to be Successful** * Flexibility, resourceful, and compassionate * Strong interpersonal skills * Self\-directed and autonomous, self\-motivated * High integrity and commitment to the mission of Faith Hospice * Passion for educating others about hospice and palliative care **Education and Experience** * Bachelor’s degree in a health care related field or equivalent * Minimum of 2 years of sales/marketing experience in a health care setting * Proven record of strong relationship building * Social media experience highly valued * Must have reliable means of transportation, valid driver’s license and provide proof of auto insurance If this rare opportunity interests you, apply today and find out more!
