**POSITION SUMMARY – LCSW Social Worker - Hospice**
**POSITION DUTIES**
– Provides psychosocial assessments, emotional support, counseling, care coordination, resource referrals, and end-of-life guidance to patients and their families to help them navigate the physical, emotional, financial, and social challenges associated with terminal illness.


**Minimum Required Qualifications –**

* CO LCSW
* 2 years recent social work experience in a healthcare setting

**PREFERRED QUALIFICATIONS**
**-**
Experience working (paid or volunteer) with the elderly


**LENGTH OF ASSIGNMENT**
– 6 weeks


**SHIFT / HOURS PER WEEK**
– Mon-Fri 8am-5pm


**START DATE**
– 10/26/2026
