Licensed Social Worker

**Seeking a Licensed Social Worker with 2-3 years’ long-term care experience to provide comprehensive psychosocial services in a hospice setting.**
**Responsibilities Include**

* Conducting initial psychosocial evaluations and ongoing counseling for patients and families
* Providing direct casework, bereavement support, and community education
* Making appropriate referrals and collaborating as a member of the interdisciplinary team

**Qualifications And Skills**

* Must hold an active license (LSW, LCSW, or equivalent)
* Experience in long-term care and hospice services preferred
* Strong communication, empathy, and teamwork skills
