**Pay: $35 –$40 per visit (based on experience)**
**Schedule: Part\-Time \| Flexible Scheduling \| PRN**

We are seeking a motivated and compassionate **Licensed Social Worker (MSW)** to join our **home health and hospice** team.
**Position Summary**
We are seeking a dedicated and empathetic **Licensed Social Worker** to join our Home Health / Hospice team. The Social Worker provides psycho\-social assessments, emotional support, care coordination, and community resource planning for patients and families facing acute, chronic, or end\-of\-life conditions.
This role is an essential part of our interdisciplinary care team.
**Responsibilities**
* Conduct comprehensive psycho\-social assessments
* Develop and implement individualized care plans
* Provide emotional support and counseling to patients and families
* Assist with advance care planning and end\-of\-life decision support
* Connect patients and families to community resources and financial assistance programs
* Participate in interdisciplinary team meetings
* Maintain accurate and timely clinical documentation
* Support crisis intervention and bereavement services (hospice)
* Ensure compliance with federal, state, and agency regulations
**Qualifications**
* Master’s Degree in Social Work (MSW) from an accredited program
* Current state licensure
* Minimum \[1 year] experience in healthcare, home health, or hospice preferred
* Strong communication and documentation skills
* Ability to work independently in a home\-based setting
* Valid driver’s license and reliable transportation
**Preferred Qualifications**
* Experience in hospice or palliative care
* Knowledge of Medicare/Medicaid regulations
* Bilingual skills (if applicable)
