We are seeking a motivated and compassionate **Licensed Social Worker (MSW)** to join our **home health and hospice** team. **Position Summary** We are seeking a dedicated and empathetic **Licensed Social Worker** to join our Home Health / Hospice team. The Social Worker provides psycho\-social assessments, emotional support, care coordination, and community resource planning for patients and families facing acute, chronic, or end\-of\-life conditions.
