**Role Description**
This is a PRN position for a Hospice Social Worker position based in San Antonio, TX. The Hospice Social Worker will provide emotional support and counseling to patients and their families, assist with discharge planning, and help navigate end\-of\-life care challenges. The role also includes coordinating resources and services, providing grief counseling, and contributing to an interdisciplinary team to develop tailored care plans.
**Qualifications**
* Master’s degree in social work from an accredited institution.
* Valid licensure or certification as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or equivalent in your state.
* Proven ability to work effectively with children, families, individuals with disabilities—including autism—and diverse populations across inpatient and outpatient settings.
* Familiarity with ICD coding systems (ICD\-9/ICD\-10\) for accurate documentation; experience with CPT coding is a plus.
* Strong skills in crisis management, behavior management, patient assessment, and medical documentation.
* Knowledge of HIPAA regulations to ensure confidentiality and privacy of patient information; experience with medical records management is preferred.
* Ability to work collaboratively as part of an interdisciplinary team
* Experience in Hospice Social Work and general Social Work practices
* Proficiency in End\-of\-Life Care and providing Grief Counseling
* Skills in Discharge Planning and connecting patients with appropriate community resources
* Strong interpersonal communication and active listening skills
Pay: $37,000\.00 \- $85,000\.00 per year
