Medical Social Worker Hospice

**What You'll Do:**

* Provides social work services in accordance with the physician’s plan of treatment.
* Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
* Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.
* Serves as a liaison with other community resources.

**Job Requirements**

* Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.
* Currently licensed to practice as a social worker in the state of **SC.**
* Minimum of one-year Hospice experience preferred.
* Valid Driver's License.
