Job Description

Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All\-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care. **Title:** Participant Care Coordinator **Location:** Lafayette, CO **Pay:** $22\.54\-$28\.91 per hour JOB SUMMARY The PACE Care Coordinator is responsible for scheduling all appointments with external mental Health providers for participants. Entering authorizations into the computer system and working with Operations Director and Finance department regarding authorizations for claims. Collaborates with mental health providers and in house mental health counselors to establish a working relationship to provide appointments and care for participants. Collaborates with PACE Medical Records office to confirm that provider notes are being received in a timely manner. III. SPECIFIC RESPONSIBILITIES 1\. Schedule all psychotherapy and psychiatry appointments for participants as ordered, timely and accurately. 2\. Creates associated transportation requests and authorizations for scheduled appointments within the EMR. 3\. Documents in the medical record as appropriate to scope of practice. 4\. Ensure coordination of appointment date/times with the participant, responsible party, and facility as appropriate and communication of the confirmation are made timely and effectively to prevent cancellations, no shows, or miscommunications. 5\. Perform any filing or clerical duties necessary to complete charting requirements. 6\. Assist in development of department processes and ensures appropriate implementation of those process to contribute to the timely and effective care of participants. 7\. Effectively communicates in clinic and team meetings as needed. 8\. Professional phone etiquette and excellent customer service skills. 9\. Demonstrate dependability through consistent compliance with scheduled work hours. 10\. Integrates processes and communication with the transportation team for coordination of both accurately. 11\. Participates in the PACE Quality Improvement (QI) program by identifying systemic opportunities for improvement, participating in quality improvement initiatives, and contributing to process improvement projects. 12\. Demonstrates interpersonal skills that promote interdisciplinary collaboration and communication. 13\. Respects the autonomy and right to confidentiality of all clients and families. 14\. Demonstrates effective communication skills with all contacts, including participants/families, IDT members, community partners, providers, and referral sources. 15\. Protects privacy and maintains confidentiality of all company procedures and information about employees, participants, and families. 16\. Follows all PACE Program Policies / Procedures and OSHA Safety Guidelines. 17\. Demonstrates ability to coordinate multiple tasks simultaneously and work independently with minimal supervision. 18\. Maintains professional affiliations and any required certifications. IV. LEVEL OF AUTHORITY AND/OR INDEPENDENT JUDGMENT This position exercises a moderate level of authority and independent judgement for determining the appropriate time for appointments in collaboration with the participant and making sure it works for their schedule and their individual health needs in collaboration with provider and IDT. V. POSITION LEVEL **EDUCATION/EXPERIENCE:** * Regular high school or holiday high school degree * 2\-3 years relevant administrative experience in medical field. * Experience working with individuals with mental health needs (Preferred). * Experience working with the frail or elderly population (Preferred). * Experience in team management, preferably in a geriatric care setting. * Experience working within an Interdisciplinary Team Model. * Experience with Electronic Medical Records (EMR). **COMPETENCIES:** * Excellent written and verbal communication skills. * Ability to work effectively with culturally, economically, and educationally diverse populations. * Must possess excellent phone \& customer service skills. * Ability to establish and maintaining effective working relationships with participants, medical staff, staff members and family caregivers. * Ability to prepare and maintain records, writing reports, and responding to correspondence. * Ability to maintain department quality assurance. * Highly proficient in organization, communication, and documentation skills. * Ability to work in a team\-based workplace environment. * Demonstrates ability to coordinate multiple tasks simultaneously, and work independently with minimal supervision. * Proficient working with spreadsheets, databases, word processing, and Electronic Health Records. * Ability to advocate effectively. * Ability to react calmly and effectively in emergency situations. * Proficient in common knowledge of safety hazards and precautions to establish a safe work environment. * Must comply with PACE health screening and immunization requirements including medical clearance for communicable diseases and up\-to\-date immunizations before having direct participant contact. **TRU's Benefits (Eligibility Varies by Position/Status):** * Paid Vacation time * 401(k) with match * Health, dental, vision \& life insurance * FSA \& HSA available * Paid orientation * Paid training * PSLF Eligible Employer * Referral \& signing bonuses (if applicable) To learn more about TRU Community Care, please visit www.trucare.org.
