**Patient Access Specialist, full\-time – Faith Hospice**
Faith Hospice has a unique opportunity for a Patient Access Specialist who feels the call to administratively support hospice patients, families, and hospice team members who serve our patients directly at end of life. This is a very rare and special gift.

At Faith Hospice, we utilize highly skilled nurses, medical social workers, chaplains, home health aides and volunteers to serve our patients, from medical and emotional care to pain management and bereavement counseling.
**What Will You Do?**
As a Patient Access Specialist, you will:
* Work a 40 hour per week schedule, Monday \- Friday.
* Receive and assist with hospice and palliative care referrals
* Schedule hospice admissions between patients/families and clinical staff
* Document in electronic medical record
* Filing/recording data
* Answer busy phones
* Participate in the holiday rotation for front desk coverage at Trillium Woods, our hospice residence

**Education and Experience**
* Position requires high school diploma or equivalent
* Our ideal candidate will have experience with electronic medical record computer systems and scheduling
* 1 to 2 years of general office processes, medical office experience is a plus
