PER DIEM HOSPICE RN

We are seeking compassionate and experienced **Per Diem Hospice RN** to provide high-quality, patient-centered hospice care throughout **Ventura and Simi Valley**.

We are looking for an RN who is clinically skilled, compassionate, dependable, and comfortable providing hospice services in patients’ homes and other community settings.

Position

* **Job Type:** Per Diem / PRN
* **Location:** Ventura & Simi Valley, California
* **Schedule:** Flexible, based on patient needs and availability
* **Experience:** **Minimum 2 years of hospice experience preferred**

Responsibilities

* Provide skilled nursing care to hospice patients in accordance with the plan of care.
* Perform comprehensive nursing assessments and monitor changes in patient condition.
* Manage pain and other symptoms and provide appropriate interventions.
* Administer medications and treatments as ordered.
* Educate patients and families regarding hospice care, medications, symptoms, and end-of-life care.
* Collaborate with physicians, hospice aides, social workers, chaplains, and other members of the interdisciplinary team.
* Communicate changes in patient condition promptly to the appropriate members of the care team.
* Complete accurate and timely clinical documentation in the electronic medical record.
* Provide compassionate emotional support to patients and their families.
* Participate in on-call or other assigned hospice visits as needed.

Qualifications

* Current and unrestricted **California RN license**.
* Current CPR certification.
* Valid driver's license, reliable transportation, and automobile insurance.
* Previous hospice experience strongly preferred; **2 years of hospice experience preferred**.
* Strong clinical assessment and communication skills.
* Ability to work independently and manage multiple patient needs.
* Compassionate, professional, and dependable approach to patient and family care.
* Knowledge of hospice philosophy, Medicare hospice requirements, and end-of-life symptom management preferred.
