PRN Hospice Chaplain

**HOSPICE CHAPLAIN** **The Hospice Chaplain meets the spiritual needs of patients/caregivers in a manner consistent with the beliefs and desires of the patient and in accordance with the patient's plan of care.** **QUALIFICATIONS:** * Ordained and endorsed minister, priest, rabbi or a member of a religious order; OR lay person with certified training in pastoral care. * Preferred experience—Direct hospice care; End\-of\-Life Care training. **DUTIES AND RESPONSIBILITIES**: * assesses the spiritual care needs of patients/caregivers; * facilitates visits by local clergy, pastoral counselors, or others who can support the patient/caregiver’s spiritual needs; * provides counseling to address the spiritual needs of the patient/caregiver in accordance with their acceptance of the service and in a manner consistent with their belief and desires; * administers requested sacraments or contacting local clergy to do so; * provides inspirational literature or music as requested; * prays with the patient/caregiver as appropriate and if requested; * assists with the planning or conducting of memorial or funeral services; and * serves as a spiritual resource to members of the IDG and members of the community at large as requested. * assesses spiritual needs every 15 days and updates plan of care; * documents all care provided in the patient’s clinical record in accordance with the hospice’s documentation requirements; * participates as a member of IDG in patient care planning.
