Registered Nurse On-Call

Select Hospice is seeking a compassionate, autonomous, and skilled Registered Nurse (RN) to join our team in an After-Hours On-Call capacity. In this vital role, you will be the clinical lifeline for our patients and their families during evenings, nights, and weekends.
The On-Call RN manages incoming phone triage, provides immediate clinical guidance, conducts urgent field visits for acute symptom management, and performs death pronouncements with warmth and dignity. We currently have a nurse taking majority of calls but want to split them or offer as many hours as you'd like if it's the right fit.

**Key Responsibilities**

* **After-Hours Phone Triage:** Respond promptly to incoming calls from patients, family caregivers, assisted living facilities, and skilled nursing facilities to assess urgent and non-urgent clinical needs.
* **Urgent Field Visits:** Conduct timely, in-person triage and bedside visits for acute symptom crises (e.g., severe pain, dyspnea, terminal agitation, nausea, or catheter/bowel issues).
* **End-of-Life Care & Pronouncements:** Perform death pronouncements, offer immediate emotional and bereavement support to families, and coordinate care with funeral homes and facility staff.
* **Interdisciplinary Collaboration:** Coordinate closely with the On-Call Hospice Physician/Medical Director to obtain urgent medication orders or treatment plan adjustments.
* **Clinical Documentation:** Maintain accurate, timely clinical notes and medication administration records utilizing electronic health record (EHR) systems in compliance with HIPAA and hospice Conditions of Participation.
* **Patient & Family Education:** Educate caregivers on comfort measures, emergency medication kit (E-kit) administration, and what to expect during the dying process.

**Qualifications & Skills**

* **Education & Licensure:** Active, unencumbered RN license in the State of Utah (or compact RN license).
* **Clinical Experience:** Minimum 1–2 years of nursing experience preferred; background in hospice, palliative care, home health, ICU, or emergency room/triage is ideal.
* **Clinical Assessment:** Strong clinical assessment and triage skills with the confidence to work independently during after-hours shifts.
* **EMR & Technology:** Proficiency with clinical documentation platforms (experience with Netsmart myUnity or similar post-acute EHR platforms is a plus).
* **Certifications:** Current CPR/BLS certification.
* **Reliability:** Valid Utah driver’s license, reliable auto transportation, active auto insurance, and willingness to travel within the Southern Utah service area.
* **Background Check:** Ability to successfully pass Utah Department of Health & Human Services (DHHS) background clearance.

**Compensation & Benefits**We value our nurses' availability and work-life balance by offering a competitive two-tier pay structure:

* **On-Call / Standby Rate:** $5.00 / hour for carrying the triage phone and remaining available to respond.
* **Active Care Visit Rate:** $45.00 / hour when dispatched for phone triage management, active clinical duties, or in-person patient visits.
* **Guaranteed Minimum Visit Pay:** **2-Hour Minimum ($90.00)** for every dispatched in-person visit, even if the visit and documentation take less than two hours.
* **Mileage Reimbursement:** Reimbursed at the standard IRS mileage rate for all patient-related travel.

Pay: $5.00 - $45.00 per hour
