**Registered Nurse (RN) Hospice Case Manager**

**Location**: Mishawaka, IN and Warsaw, IN | Full-Time

**Compensation**: Top-Tier Rates (Up to 90th Percentile) + Mileage Reimbursement + Potential for Bonuses

**The Role**: As a Hospice Case Manager, you are the clinical anchor for your assigned caseload. You will:

**Manage Total Care**: Perform comprehensive clinical assessments, manage pain and symptoms, and adjust plan-of-care orders in real-time with our local physicians.

**Lead the Interdisciplinary Team**: Coordinate closely with social workers, chaplains, aides, and medical directors to ensure seamless, high-touch support for families.

**Advocate & Educate**: Guide patients and caregivers through end-of-life transitions with dignity, clear communication, and compassionate hands-on care.
