### **Responsibilities**

As a Hospice Visit Nurse, you will:

* Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing evaluations.
* Develop, implement, and update individualized plans of care based on patient needs.
* Collaborate closely with the hospice physician and full Interdisciplinary Team to ensure appropriate interventions.
* Oversee and support the work of home health aides, social workers, and chaplains.
* Educate patients and caregivers to promote safety, comfort, and understanding.
* Maintain high standards of documentation, compliance, and clinical excellence.
* Contribute to continuous improvement efforts to elevate the quality of care across the agency.

### **Qualifications**

* Active **PA Registered Nurse (RN)** license
* Minimum **1 year of RN experience**
* Willingness to travel within assigned service areas
* Valid driver's license, reliable transportation, and active auto insurance
* Strong communication and interpersonal skills

**Job Type:** Part-Time / Per Diem
**Pay:** $50.00 – $60.00 per hour

**Work Location:** In the Community – Patient Residences (Travel Required)
