RN Case Manager

**About the Role:**
We are seeking a compassionate and highly organized **Registered Nurse Case Manager** to join our hospice care team. This role is ideal for a nurse who values a **holistic approach to end-of-life care**, thrives in a **team-oriented environment**, and is dedicated to providing comfort, dignity, and support to patients and their families. We have 2 positions available: Eastside Tucson and Northwest Tucson.

**Key Responsibilities:**

* Manage a caseload of hospice patients, coordinating care plans and services.
* Conduct comprehensive assessments and develop individualized care plans.
* Provide skilled nursing care, symptom management, and emotional support.
* Collaborate with interdisciplinary team members including physicians, social workers, chaplains, and aides.
* Educate patients and families on disease progression, medications, and care techniques.
* Maintain accurate documentation and ensure compliance with hospice regulations.
* Participate in on-call rotation as needed.

**Qualifications:**

* Current RN license in Arizona.
* Minimum of 1 year of nursing experience; hospice or palliative care experience preferred.
* Strong organizational and time management skills.
* Excellent communication and interpersonal abilities.
* Ability to work independently and as part of a collaborative team.
* Commitment to compassionate, patient-centered care.

Job Type: Full-time

Pay: $80,000.00 - $90,000.00 per year
