RN Case Manager - Hospice - South Orange County - Hospice Experience Required

We are looking for a hospice experienced and professional, full-time Hospice RN Case Manager to care for our patients in the South Orange County area.

**Immediate Hire Position**

Job Duties

* **Assess Patients:** Evaluate pain, physical symptoms, and emotional needs regularly.
* **Create Care Plans:** Design and update personalized care plans with the medical team.
* **Manage Symptoms:** Administer medications and treatments to keep patients comfortable.
* **Educate Families:** Teach family members how to care for the patient and what to expect.
* **Supervise Staff:** Direct certified care team
* **Coordinate Services:** Arrange medical equipment, supplies, and visits from social workers or chaplains.
* Attend care plan meetings (IDG/IDT)

Skills Needed

* **Clinical Expertise:** Strong knowledge in pain management, wound care, and palliative nursing.
* **Compassion:** High empathy and patience to support grieving families.
* **Communication:** Clear speaking and active listening skills to explain complex medical issues simply.
* **Time Management:** Ability to organize a busy schedule and prioritize urgent patient needs.
* **Critical Thinking:** Quick decision-making skills during sudden health changes.
* Must have at least 3 years of experience as a Hospice RN Case Manager

Job Type: Full-time, 40 hours per week with paid overtime

Hourly: $50-55 per hour

Paid mileage @ .72 cents per mile

* Visiting patients private homes, board and care and rehab facilities

Pay: $50.00 - $55.00 per hour
