Hospice Registered Nurse (RN) – Contingent Nights
We are seeking a compassionate and skilled Hospice Registered Nurse (RN) to join our team on a contingent night\-shift basis in our in\-patient Hospice Facility. In this rewarding role, you will provide high\-quality end\-of\-life care to patients with terminal illnesses while offering guidance, comfort, and support to families during challenging times. The ideal candidate is clinically strong, empathetic, organized, and committed to delivering patient\-centered hospice care.
Position Summary
The Hospice RN provides compassionate nursing care to hospice patients using the nursing process, with a focus on comfort management, symptom relief, and quality of life. This role collaborates closely with physicians, families, caregivers, and the interdisciplinary team to ensure individualized care plans are implemented and updated appropriately.
Essential Responsibilities
* Perform comprehensive patient assessments related to terminal diagnoses and develop individualized plans of care.
* Determine appropriate frequency and duration of nursing services based on patient acuity and family needs.
* Assess the need for interdisciplinary services and initiate referrals as appropriate.
* Administer and document treatments and interventions according to physician orders.
* Provide ongoing assessment of physical, emotional, psychosocial, spiritual, and environmental patient needs.
* Communicate changes in patient condition to physicians and interdisciplinary team members.
* Educate patients and families regarding disease progression, symptom management, end\-of\-life care, and ethical considerations.
* Assess families for bereavement risk, cultural concerns, spiritual needs, and educational support.
* Develop and oversee hospice aide care plans with clear goals and interventions.
* Supervise LPNs, hospice aides, CNAs, and paraprofessional staff in accordance with regulatory requirements.
* Conduct supervisory visits for hospice aides at least every 14 days and annual onsite visits as required.
* Apply hospice admission and recertification criteria to determine patient eligibility and level of care.
* Maintain accurate and timely clinical documentation, progress notes, and chart reviews.
* Participate in interdisciplinary team (IDT) meetings and quality improvement initiatives.
* Utilize electronic systems including RXNT and Enclara.
* Support organizational policies, procedures, and compliance standards.
* Assist with Hospice Home Supervisor responsibilities as needed in their absence.
Additional Responsibilities
* Attend all required trainings, meetings, and in\-service programs.
* Maintain strict confidentiality of patient, employee, and agency information.
* Foster positive working relationships through professional communication and teamwork.

* Perform other duties as assigned.
QualificationsEducation \& Experience
* Graduate of an accredited school of nursing required.
* Hospice, home health, palliative care, or end\-of\-life care experience preferred.
* Experience with behavioral health issues or lived experience is a plus.
Licensure \& Certifications
* Current, unencumbered Registered Nurse (RN) license in the State of Michigan required.
* Current CPR/BLS certification required.
* CHPN or hospice/palliative care certification preferred.
