**About the Role:**

Solara Hospice LLC is looking for a compassionate and dedicated Registered Nurse (RN) to join our team. **Responsibilities:**

* Provide skilled nursing assessments and direct patient care to hospice patients in home or facility settings
* Develop, implement, and evaluate individualized patient care plans in coordination with the interdisciplinary team
* Manage pain and symptom control to maximize patient comfort and quality of life
* Educate patients and family members on disease progression, medications, and end-of-life care
* Communicate and collaborate with physicians, social workers, chaplains, and other care team members
* Document patient assessments, care plans, and clinical notes accurately and in a timely manner
* Provide emotional support and guidance to patients and families throughout the hospice journey

**Requirements:**

* Active Registered Nurse (RN) license in the state of Massachusetts
* Previous hospice, palliative care, or home health nursing experience preferred
* Strong clinical assessment and critical thinking skills
* Excellent communication and interpersonal skills with patients, families, and care teams
* Ability to work independently and manage a patient caseload efficiently
* Proficiency with electronic medical records (EMR) documentation
* Valid driver's license and reliable transportation for patient visits
