Social Worker

Compassionate Hospice is looking for an experienced Hospice Social Worker MSW to assist with our patients in the Denver Metro area and North suburbs. Compassionate Hospice Care is a high quality organization certified by CHAP. Mileage and Expense Reimbursement: Employee travel is paid out based on company pay practices $0.70/mile.

Pay Range: $64,388.30 - $68,369.31 per year

Schedule: Monday- Friday 9:00-5:00

Requirements for Hospice Social Worker:

· Minimum Education: Master’s degree in social work from an accredited school of Social Work.

* Minimum Experience: One (1) year of social work experience in a hospice setting.

· Required License: Current Colorado Social Worker license LSW or LCSW licensure.

· Valid Colorado state driver’s license and proof of auto insurance.

**Responsibilities** **for** Hospice Social Worker:

· Provides clinical psychosocial support to patient and family/caregiver in accordance with patient preferences, goals, and needs. The position works collaboratively with the Interdisciplinary Team/Group (IDT/IDG) as well as with facilities and agencies in the community.

* Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required.
* Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high-risks for suicide, neglect or abuse and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Provides crisis intervention when necessary.
* Completes initial and on-going psychosocial and bereavement assessment of patient and family/caregiver to include the patient’s history, ethnic background, cultural and religious beliefs and the implications for the Plan of Care (POC).
* Communicates risk indicators to bereavement services as appropriate. Assists RN Case Manager with development of a comprehensive transfer and/or discharge plan in collaboration with patient, family, staff and referral agencies.
* Evaluates patient/family psychosocial needs and individual/family counseling as part of the Plan of Care (POC). Delivers appropriate referrals to community resources such as financial assistance, legal aid, housing and/or education.
* Identifies potential bereavement resources for patient and family/caregivers to assist in coping with loss, aging and death. Assesses and assists patients with advanced directives and care planning.

Pay: $65,000.00 - $72,500.00 per year
