Social Worker - LMSW

We are seeking an experienced **Hospice Social Worker (MSW)** to join our interdisciplinary team and provide compassionate psychosocial support to patients and families navigating end\-of\-life care. This role delivers coordinated care in the home setting and serves as an advocate, counselor, educator, and resource for patients and caregivers. ### **As a Hospice Social Worker, you will:** * Complete and document the **Comprehensive Assessment within 5 days of hospice admission** to evaluate psychosocial, emotional, financial, environmental, and community resource needs. * Develop and implement interventions that support patient goals and align with the individualized **Plan of Care (POC)**. * Provide counseling and emotional support related to **end\-of\-life care, grief, loss, family dynamics, and adjustment**. * Assist patients and families with **advance directives, funeral planning, discharge planning, and care transitions**. * Coordinate community resources and connect families with available support services. * Participate as an active member of the **Interdisciplinary Group (IDG)** through collaboration and care planning. * Maintain timely and accurate clinical documentation. * Promote dignity, respect, patient rights, and compassionate service throughout the care experience. ### **Qualifications:** ✔ **Master’s Degree in Social Work (MSW) from an accredited program** ✔ **Current Social Worker License** ✔ **Minimum 1 year of Hospice Social Work experience required** ✔ Strong understanding of hospice philosophy and end\-of\-life care ✔ Valid Driver’s License and current Auto Insurance ✔ Current **AHA CPR Certification** * HomeCare HomeBase experience preferred
