**Social Worker, Master’s – Hospice Home Visits, full\-time**
Faith Hospice is seeking a full\-time Master's prepared Social Worker to join our team and provide support to patients, families, and caregivers by offering emotional support, information, education, long\-range care planning, short\-term counseling, and assistance with identifying and accessing resources.
**As a Hospice Social Worker, you will:**
* Conduct thorough assessments of social and emotional factors/issues:
o Family dynamics
o Financial and environmental issues
o Bereavement needs
o Patient/family/caregiver goals and communication styles
o Develop and implement the care plan in conjunction with the interdisciplinary group (IDG)
* Work a Monday – Friday schedule, 8a – 5pm with participation in the after hours on\-call rotation with the Hospice MSW team
**The Hospice Social Worker role requires:**
* Master’s degree in social work from an accredited school
* Full licensure from the State of Michigan as an MSW (limited license is accepted with the requirement of continuing to work towards full licensure while employed by Faith Hospice)
* Due to the autonomous nature of this position, it also requires a minimum of one year of social work experience in a health care/medical setting with strong knowledge of community\-based resources.
* Prior hospice social work experience is highly valued.
* Candidates must also have proof of Michigan driver's license and vehicle liability insurance as well as a reliable means of transportation to make home visits.
